Job Description:

We are looking to add a Social Worker to our hospice team to serve patients in the Boulder/Longmont area.

Primary Duties and Responsibilities:

• Performing initial and ongoing social worker assessments per standards of practice on each assigned hospice patient and developing the plan of care in collaboration with the patient/representative, attending physician and interdisciplinary team members.
• Using a framework of assessment, implementation, coordination and evaluation providing outcome-based services for an assigned caseload from admission to discharge. Establishing goals and providing social services to address patients physical and psychological needs.
• Helping build our hospice bereavement & volunteer programs.

Required Qualifications:

• Have a Master of Social Work degree from a school accredited by the Council on Social Work Education
• Be currently licensed as a Social Worker in Colorado - Licensed Social Worker (LSW) or Licensed Clinical Social Worker (CSW) is required.
• Have prior experience with patient and family Medicare & Medicaid navigation

Compensation: $30-$45/hour
